The Provider Portal is a tool for Provider agencies, to update the information about your agency, which appears in the NY Connects Resource Directory. Through this Portal, we may also share various Announcements that you may find helpful.

Please contact the local or state agency that you currently contract or conduct business with to obtain access or to have changes made to your listing:

• If you are a Provider agency offering long term services and supports in New York but are not currently listed in our NY Connects Resource Directory and would like to be added, or
• If you are an existing Provider agency listed in the NY Connects Resource Directory and would like to make changes to your information.
